The result and match official marks form shall be completed by both teams and must be received by the NFA either by e-mail to cupcompetitions@nottinghamshirefa.com or by post, within 72 hours of the match being played.
Both teams will need to mutually agree on a new date, if a new date can't be met Nottinghamshire FA will be notified and we will arrange a new date.
